Application Guidelines
If Funded
Individual Application
Dear Applicant:
New England Biolabs Foundation will be
pleased to evaluate your request on the basis of the following
data and supporting materials:
1. A cover letter stating for what
specific purpose you require fund and in what amount. Please
itemize. Please list your goals for this project and the ways
you plan to attain them.
2. A minimum of two written professional
references.
3. Any material relevant to the grant
(pictures, tapes, media articles, etc.)
4. Curriculum Vitae.
5. The following financial information:
• Current annual income from all
sources, taxable or tax-free, earned or received, for each of
the last two years (broken down according to salaries,
interest, rents, etc.);
• List of assets for the last two
years. Please list professional (supplies and equipment) and
non-professional supplies separately.
• Current annual professional
expenses. Please itemize.
• Please indicate whether or not
your spouse or other family members contribute to your income.
If so, please specify the name, relationship and income or
amount contributed to you.
• Monthly total rent or mortgage
payments for your home and/or studio.
• Bank references. (names, addresses
and telephone numbers.)
6. Other grants you have applied for or
received in the last two years.
Further financial data may be requested,
if necessary.
You may be assured that this information
will be held in the strictest confidence.
Please include the following signed
statement:
“I affirm that the information
provided in this application is true and accurate to the best
of my knowledge. I give permission to New England Biolabs
Foundation to contact appropriate persons or organizations to
verify or clarify information in this application.”
